ABSTRACT

Dos novelas sucesivas del escritor y matemático argentino Guillermo Martínez, "Crímenes imperceptibles" (2004) y "La muerte lenta de Luciana B." (2007) fueron llevadas al cine. En ambas se abordan las categorías lógico-matemáticas de azar y necesidad, permitiendo abrir una brecha entre ellas para conjeturar una hipótesis clínica sobre la responsabilidad subjetiva de los personajes. Este trabajo pone el foco en la versión "La ira de Dios" (Schindel, 2022), introduciendo la cuestión de las leyes del azar, la respuesta de un sujeto a ese encuentro con el azar, para concluir con la venganza como rechazo de la responsabilidad.


Two successive novels by Argentine writer and mathematician Guillermo Martínez, "Imperceptible Crimes" (2004) and "The Slow Death of Luciana B." (2007) were made into movies. In both, the logical-mathematical categories of chance and necessity are addressed, allowing a gap to be opened between them to conjecture a clinical hypothesis about the subjective responsibility of the characters. This work focuses on the version "The Wrath of God" (Schindel, 2022), introducing the question of the laws of chance, the response of a subject to that encounter with chance, to conclude with revenge as a rejection of the responsibility

This paper studies the necessity of the current legislation on the supervision of medical devices in China from the perspectives of strengthening administration according to law, protecting public health, perfecting the legal system of medicine and promoting the development of the medical device industry. This study analyzes and summarizes the legislative experiences and forms in the field of medical device regulation in the United States, the European Union, Japan and other countries and regions, at present, the conditions of carrying out the legislation of medical device supervision in China are quite mature, and some policy suggestions are put forward for the enactment of the law of medical device management in China.

ABSTRACT

Introducción: la enfermedad por COVID-19 genera el síndrome de distrés respiratorio agudo, afecta la necesidad de oxigenación y demanda cuidados de enfermería para mantener la estabilidad hemodinámica y prevenir complicaciones respiratorias. Objetivo: analizar la evidencia científica sobre los cuidados de enfermería ante la necesidad de oxigenación en adultos hospitalizados con enfermedad por COVID-19. Métodos: revisión integrativa con un universo de 518 artículos científicos de 2020 y 2021 de las bases de datos: PubMed, Ebsco, Cuiden y ScienceDirect. Se analizaron seis (n= 6) estudios que abordan: ¿cuáles son los cuidados de enfermería ante la necesidad de oxigenación en adultos con enfermedad por COVID-19? Resultados: los cuidados de enfermería incluyen estrategias de oxigenoterapia, recomendaciones sobre el cuidado en la primera línea de atención, uso de oxígeno de alto flujo, posicionamiento prono despierto temprano, y control de saturación y de signos vitales mediante un proceso de atención en enfermería. Conclusión: los cuidados de enfermería satisfacen la necesidad de oxigenación mediante intervenciones de oxigenoterapia, control de comorbilidades y prevención de riesgos hospitalarios.


Introduction: COVID-19 disease generates acute respiratory distress syndrome, affects the need for oxygenation and demands nursing care to maintain hemodynamic stability and to prevent respiratory complications. Objective: To analyze the scientific evidence on nursing care facing the need for oxygenation in hospitalized adults with COVID-19 disease. Methods: Integrative review of a universe of 518 scientific articles published from 2020 to 2021 in the following databases: PubMed, EBSCO, CUIDEN and ScienceDirect. N=6 studies were analyzed. These address what the nursing care implies facing the need for oxygenation in adults with COVID-19. Results: Nursing care includes oxygen therapy strategies, recommendations on care in the first line of care, use of high-flow oxygen, early awake prone positioning, control of saturation and vital signs, through a nursing care process. Conclusion: Nursing care satisfies the need for oxygenation through oxygen therapy interventions, control of comorbidities and prevention of hospital risks.


Introdução: a doença Covid-19 gera síndrome do desconforto respiratório agudo, afeta a necessidade de oxigenação e demanda cuidados de enfermagem para manter a estabilidade hemodinâmica e prevenir complicações respiratórias. Objetivo: analisar as evidências científicas sobre o cuidado de enfermagem diante da necessidade de oxigenação em adultos hospitalizados com doença Covid-19. Métodos: revisão integrativa; universo 518 artigos científicos de 2020-21, nas bases de dados: PubMed, Ebsco, Cuiden e ScienceDirect. Foram analisados N=6 estudos que abordam quais são os cuidados de enfermagem diante da necessidade de oxigenação em adultos com Covid-19? Resultados: a assistência de enfermagem inclui estratégias de oxigenoterapia, recomendações sobre cuidados na primeira linha de cuidado, uso de oxigênio de alto fluxo, posicionamento prono acordado precocemente, controle da saturação e dos sinais vitais, por meio de um processo de cuidado de enfermagem. Conclusão: a assistência de enfermagem satisfaz a necessidade de oxigenação, por meio de intervenções de oxigenoterapia, controle de comorbidades e prevenção de riscos hospitalares.

ABSTRACT

It has been proposed that age reprogramming enables old cells to be rejuvenated without passage through an embryonicstage (Singh and Zacouto in J. Biosci. 35 315–319, 2010). As such, age reprogramming stands apart from the inducedpluripotent stem (iPS) and nuclear transfer-embryonic stem (NT-ES) cell therapies where histo-compatible cells are produced only after passage through an embryonic stage. It avoids many of the disadvantages associated with iPS and NT-EScell therapies. Experimental evidence in support of age reprogramming is burgeoning. Here, we discuss possible newapproaches to enhance age reprogramming, which will have considerable benefits for regenerative therapies.

ABSTRACT

Introducción: el conocimiento sobre reperfusión coronaria en el infarto agudo del miocardio con elevación del segmento ST en el nivel primario de atención es de extraordinaria importancia para disminuir las complicaciones y la mortalidad. Objetivo: determinar las necesidades de aprendizaje de médicos de atención primaria sobre reperfusión coronaria en el infarto agudo del miocardio. Método: se realizó un estudio descriptivo de 65 galenos que laboraban en 5 policlínicos principales de urgencias de Santiago de Cuba en noviembre del 2017, para lo cual se aplicó una encuesta con variables de interés, que se calificó según metodología de evaluación y estándar previamente establecida por el equipo de trabajo. Resultados: del total de preguntas, solo una (5,9 por ciento) cumplió con el estándar de más de 70 por ciento de médicos con respuestas adecuadas; los años de experiencia y haber recibido adiestramiento influyeron positivamente en el número de dichas respuestas. La falta de capacitación fue la causa por la cual no se aplicó el tratamiento, y la planificación de esta, así como la creación de algoritmos de trabajo resultaron las medidas más propuestas por los galenos. Ninguno de ellos refirió sentirse adiestrado correctamente. Conclusiones: los médicos que laboraban en los 5 policlínicos principales de urgencias mostraron necesidades de aprendizaje y capacitación sobre el proceso de reperfusión coronaria.


Introduction: the knowledge on coronary reperfusion in the acute myocardial infarction with elevation of the ST segment in the primary care level is of extraordinary importance to decrease complications and mortality. Objective: to determine the necessities of doctors from primary care on coronary reperfusion in the acute myocardial infarction. Method: a descriptive study of 65 physicians who worked in 5 main polyclinics for emergencies of Santiago de Cuba was carried out in November, 2017 for which a survey with variables of interest was applied which was qualified according to evaluation methodology and previously established standard by the working team. Results: of all questions, only one (5.9 percent) fulfilled the standard of more than 70 percent of doctors with appropriate responses; the years of experience and receiving a previous training influenced positively in the number of these responses. The lack of training was the cause for which the treatment was not applied, and its planning, as well as the creation of working algorithms were the measures mostly proposed by the physicians. None of them referred to be correctly trained. Conclusions: doctors who worked in the 5 main emergency polyclinics showed learning necessities and training on the management for coronary reperfusion.

ABSTRACT

Although 19 years have passed since the enforcement of the Framework Act on Health and Medical Services, the Plans for Development of Health and Medical Services has not been established. This Plan is a 5-year basic long-term plan that covers the whole of health and medical services. This Plan should point to the direction of 30 long-term plans of healthcare, and this Plan should serve as a combination and coordination of 30 long-term plans and 22 related laws. The United States, the United Kingdom, and Japan have established long-term healthcare plans (4-, 10-, and 20-year plans, respectively). The long-term health plan of the United States has been approached bottom-up, those of the United Kingdom and Japan have been approached top-down. The rapid environmental changes that Korea is and will be experiencing emphasize urgently the need for establishing the Plan for Development of Health and Medical Services.

The prevalence of allergic diseases in children have increased year by year recently.Although the clinical manifestations of childhood allergic diseases are various, their pathogenesis is nearly the same.In the meantime, childhood allergic diseases are combined with nutritional, psychological, spiritual and family social problems, which if not solved properly will affect the allergic diseases themselves.Therefore, it is necessary to apply the concept of holistic integrated medicine and integrate multiple related departments to solve the problem of allergy, which is patient-oriented.This article introduces the necessity and approaches for integration of childhood allergic diseases.

Compared with acupuncture research, moxibustion research has made slow progress with great obstacles. With randomized controlled trials as the widely accepted gold standard for the evaluation of clinical effect, moxibustion therapy is facing a great challenge: how to establish a reasonable placebo control group. This article reviewed the current application of placebo moxibustion, analyzed the difficulties in setting up placebo moxibustion, and explored corresponding solutions, in order to attract more attention to moxibustion therapy among traditional Chinese medicine practitioners.

Glaucoma is the leading cause of irreversible blindness worldwide.Screening for glaucoma is a critical intervention to identify individuals at an earlier stage of the disease.However,there is controversy about the necessity of glaucoma screening.Screening is a critical intervention for primary prevention of disease.The screening which suited to the level of our economic development has important benefits for our country,society,families and individuals.Glaucoma has fulfils the criteria of WHO for population screening:(1) Glaucoma is a major public health problem in the world.(2) Glaucoma can be detected in its earlier stage.(3) There are suitable and acceptable examinations for glaucoma screening.(4) Early detection and subsequent treatment could delay or retard glaucoma disease progression.(5) With appropriate strategy design,glaucoma screening can creat good economic benefit.The screening strategies depend on the level of social economy development,the health policies of developed areas tend to prevent disease in its early stages.In poor areas,the limited medical resources are used to treat patients in the advanced stages.In the relatively developed area with complete health care facilities and incomplete referral mechanisms,screening for glaucoma is a critical intervention to identify individuals at an earlier stage of the disease.Glaucoma Screening project in Wenzhou suggested that screening should be implement in relatively developed regions of high density of population to improve efficiency;screening should aim for multiple eye diseases to lower the cost per unit benefit.

Objective:To implement quality control in whole life cycle for infant incubator, and ensure it working in safe and efficient situation.Methods:The parameter detection cases of infant incubator was analyzed and researched according to relative standards and requirements of technique for medical equipment.Results:The detection data and frequent failures of clinical cases revealed that there were parameter deviation existing in infant incubators. These deviations should be adjusted in time according to relative standards and requirements of technique for medical equipment.Conclusion:The quality control of whole life cycle for infant incubator is the necessary condition for ensuring medical quality and safety of patients. To pay attention to the quality control before and after application and after maintenance of the infant medical incubator, in order to ensure the medical equipment can achieve to meet the performance requirement of treatment and provide safe and efficient quality service for patients.

ABSTRACT

A questionnaire-based survey was administered to determine how community pharmacists recognize the necessity for and frequency of touching their patients during care giving. The questionnaire was sent to 400 community pharmacies that practiced home care, and 147 valid responses were analyzed. The survey suggested that acts recognized by pharmacists to require touching were measuring vital signs and helping patients take oral medications. It was thought that pharmacists needed to help patients in taking oral medications and also needed to determine issues related to oral intake. The survey also indicated that the frequencies of measuring vital signs and applying plasters to patients were high. Measuring vital signs was considered routine work, and applying plasters to patients was considered occasional work. Many respondents stated the need for practical study sessions on measuring vital signs. The frequency of measuring vital signs has increased because pharmacists have increasingly recognized this need. Additionally, females tended to touch their patients more frequently than males. However, there was no relationship between the frequency of examining bedsores and applying ointments on them and gender because it was thought that knowledge and experience were more important. The recognition of necessity for touching their patients was high but the frequency was comparatively low, and there was a discrepancy between the recognition of necessity and frequency. It was suggested that practical study sessions on when patients should be touched should be a high priority.

El film Unthinkable relata el tratamiento que el ejército norteamericano le da a un ex agente de la CIA convertido al islamismo que dice haber escondido tres bombas nucleares en distintas ciudades importantes de los EEUU. El problema planteado al comienzo del film es cómo torturar al prisionero para obtener la información necesaria para desactivar las bombas a tiempo.El film sirve como excusa para presentar la legislación internacional que prohíbe la tortura, así como las leyes especiales promulgadas a partir de los atentados del 11 de septiembre contra las torres gemelas. Asimismo se presentan la relación compleja entre ley, estado de necesidad y estado de excepción, relación a partir de la cual se dan las condiciones de excepcionalidad a las leyes que prohíben la tortura.La presentación de la legislación internacional permite entender los motivos que hacen que los discursos sostenidos por los distintos personajes del film tengan más o menos fuerza. A partir del análisis esos discursos se analiza el modelo de héroe presentado en la película y su funcionalidad a la “guerra contra el terror” desatada por la administración Bush.Finalmente se realiza un análisis de las condiciones de posibilidad que llevaron a la American Psychological Association a elaborar un documento que permite y avala la participación de psicólogos en los interrogatorios a prisioneros en la cárcel de Guantánamo o en Abu Ghraib...


The film Unthinkable functions as a pretext to present international legislation that prohibits the use of torture as well as the specific laws enacted after the 9/11 terrorist attack on the twin towers. Furthermore, this article presents the complex relationship between the law, necessity and state of emergency, relationship that determines the conditions for the exceptions to the laws that prohibit the use of torture. The international legislation presented allows for a better understanding of the motives that make the discourses of different characters in the film more or less strong. By analyzing these discourses, this article attempts to study the heroic model presented in the movie and its possible function in the "war on terror" unleashed by the Bush administration. Finally, this paper analyzes the specific conditions that led the American Psychological Association to dictate a document that not only allows, but also supports, the participation of psychologists in the interrogation of prisoners at Guantanamo Bay or Abu Ghraib

Along with the degree of population ageing worse off continuously, the demand for medical service is growing. The current system of pension service and medical service are discrete and separate, and integrating pen-sion service with medical service is the best choice for China. However, there are some problems with the model for integrating pension service with medical service. These problems include the lack of funding, negative participation and imperfect methods of service delivery; high toll levels and single, rigid service contents; and bull management and policies hard to enforce. China should improve the service delivery model, identify service content according to the demand for service object, and improve the mechanism of government management.

From the angle of global health talent need , this article deeply illuminated that the global health talent training was helpful to take advantage of international resources to solve the problem of health, serve China's peaceful development strategy, comply with development trend of public health, and remedy limitations of traditional medicine undergraduate. Training undergraduates in global health who are China-specific and global competent is the only way to meet the talent demand of China's future. Multi-level global health education will become an important part of medical education in future in China.
